Because of the environmental conditions, the involvement of Southwestern Adventist University, Earth History Research Center, and the Hanson Research Station, there are two paper forms and one form for unaccompanied minors that each participant needs fill out, sign, and file with the project leaders before staying at Camp Cretaceous overnight or any actual participation in the quarrying process is permitted.
All of the information supplied will be held in the strictest confidence.
All these forms are available when you arrive at the project site. However, you may wish to download them, fill them out, and bring the signed forms with you. The one form which may request information that you may not remember details is the Southwestern and Earth History Medical Information Form. In particular, you may find it convenient to download at least this one form and fill it out before leaving home!
If you are minor and not arriving with a parent or guardian, then you will need to download both of the above forms and the Southwestern Consent to Treat Minor, fill all FIVE out, sign them, have a parent or guardian also sign them, and bring them with you.
No work in a quarry is permitted until ALL required forms have been properly filled out, signed, and filed with the project leaders.
